News Topical, Digital Desk : Under the Intensive Revision Program 2026, Uttar Pradesh requires filing a declaration form along with Form 6, which includes the applicant's name and address, spelled correctly, along with a recent photograph and mobile number. This information was provided by the State Election Commission in a press release. 

For age proof, birth certificate, Aadhaar, PAN, driving licence, educational certificate or passport will be valid, whereas in the absence of documents, affidavit as per the prescribed age group or certificate issued by the local body will be accepted, whereas for residence proof, Aadhaar, one year old electricity-water-gas bill, bank passbook, passport, land records, lease deed or sale deed will have to be given and in their absence, on-site verification will be done.

The press release said that in the declaration form, the details of self or family members from the voter list of the year 2003 will have to be filled, in case of non-availability of details, a notice will be issued, in response to which the prescribed documents related to date of birth and place of birth will have to be submitted, whereas eligible citizens can submit Form-6 and declaration online at voter.eci.gov.in or offline through BLO/ERO.

Chief Electoral Officer Navdeep Rinwa stated that a declaration form must be completed along with Form 6, which citizens fill out under the Special Intensive Revision Program 2026. Form 6 must include the applicant's name and correct address, a clear recent photograph, and a current mobile number.

What documents will be required for age certificate in UP SIR?

· Birth certificate issued by the competent authority
· Aadhar Card
· PAN Card
· Driving License
· High School/Intermediate certificate mentioning the date of birth
· Indian Passport
· Any other document for proof of date of birth (if above documents are not available)

UP SIR: What to do if you don't have these documents?

· In such cases, applicants in the age group of 18 to 21 years will have to appear before the Booth Level Officer/Assistant Electoral Registration Officer/Electoral Registration Officer with an affidavit (Annexure-27) signed by their mother/father or Guru (referring to the third gender).
· If none of the above documents are available and the mother/father is also not alive, then the applicant can submit the age certificate issued by the village head/members of Municipal Corporation/Nagar Panchayat.
· If the age of the applicant is 21 years or more, then he/she will have to appear in person before the Booth Level Officer/Assistant Electoral Registration Officer/Electoral Registration Officer and submit only his/her declaration regarding age.

Which documents are required for residence in UP SIR?

· Water/Electricity/Gas connection bill (at least one year old) at that address
· Aadhaar Card
· Current Pass Book of National/Scheduled Bank/Post Office
· Indian Passport
· Land ownership record from Revenue Department including Kisan Bahi
· Registered Rent Lease Deed (in case of tenant)
· Registered Sale Deed (in case of own house)

In case any of the above records is not available, it will be necessary to get the on-site verification done by the Electoral Registration Officer.

A declaration is also mandatory with Form 6 submitted under the Special Intensive Revision-2026. The applicant must submit the declaration form along with their own details, those of their parents, grandparents, or one of their grandparents, from the final voter list of 2003, along with the Assembly constituency number, part number, and serial number. In the event of correct mapping, no notice will be issued to the applicant. If the details provided in the declaration form are not available or do not match the previous Special Intensive Revision Electoral Roll-2003, a notice will be issued to the applicant.

If a notice is received in UP SIR, how will you respond?

1- Any Identity Card/Pension Payment Order issued to a regular employee/pensioner of any Central Government/State Government/Public Sector Undertaking.
2- Any Identity Card/Certificate/Record issued in India by Government/Local Authorities/Banks/Post Office/LIC/Public Sector Undertaking before 01.07.1987.
3- Birth Certificate issued by Competent Authority.
4- Indian Passport
5- Matriculation/Educational Certificate issued by Recognized Boards/Universities
6- Permanent Residence Certificate issued by Competent State Authority
7- Forest Rights Certificate
8- OBC/SC/ST or any Caste Certificate issued by Competent Authority
9- National Register of Citizens (wherever applicable)
10- Family Register prepared by State/Local Authorities
11- Any Land/House Allotment Certificate issued by Government.
12- For Aadhaar, the Commission's instructions issued vide letter No. 23/2025.ERS/Vol.II dated 09.09.2025 (Annex II) will be applicable.
13- Extract of the electoral roll of Bihar SIR as on the reference date of 01.07.2025.

It was stated that if an applicant was born in India before 01.07.1987, then one of the 13 above mentioned documents must be provided as proof of date/place of birth. Similarly, if an applicant was born in India between 01.07.1987 and 02.12.2004, then in addition to providing one of the 13 above mentioned documents as proof of date/place of birth, a record of the father's or mother's date/place of birth from the above list must also be provided.

If the applicant was born in India after 02/12/2004, one of the 13 documents above must be provided as proof of their own date/place of birth, as well as proof of the date/place of birth of both the father and mother. If neither parent is an Indian citizen, a copy of that parent's valid passport and visa at the time of the applicant's birth must be provided.

If the applicant was born outside India, then Birth Registration Certificate issued by Indian Mission abroad must be enclosed and if Indian citizenship has been acquired by Registration/Naturalisation then Citizenship Registration Certificate must be enclosed.

Any eligible citizen can apply by visiting the Election Commission of India's portal, voter.eci.gov.in, and filling out Form 6 (with declaration) online. Offline, Form 6 can be accurately completed, along with the declaration, and submitted along with the necessary documents to their BLO/Assistant Electoral Registration Officer/Electoral Registration Officer.
