Moving beyond standard contraception and birth-spacing programs, state-run medical facilities will now offer specialized pre-conception counseling alongside direct infertility treatments for couples experiencing challenges conceiving. Demographic Shift: Dropping Fertility Rates and a Rising Senior Population

This healthcare expansion responds directly to shifting demographic realities across the state. Official health data indicates that annual births in Tamil Nadu dropped from approximately 9.39 lakh in 2020 to around 8.02 lakh by 2025. Concurrently, the state’s total fertility rate (TFR) has contracted to 1.3 children per woman—well below the national replacement benchmark of 2.1 required for long-term population stability. At the same time, senior citizens aged 60 and older currently account for roughly 16 percent of the state’s population, a proportion projected to reach 20 percent by 2036.
